Forms Page

1.    The forms are the same as last year so please disregard dates. Forms 77-26, 77-27 and 77-28 are applicable for the dates of July 1, 2008 through June 30, 2009. 

2.    Please look under 8 Support Sites at www.ilnbpts.org for your ROE/ISC Regional Support Site Number that is required on the forms. If you are CPS, please put CPS.

IMPORTANT: Forms 77-26, 77-27, 77-28 are due May 1st, 2009

VERY IMPORTANT:
Each time you document 30 hrs please submit Forms 77-26, 77-27, 77-28 A.S.A.P.!

Check the lists at www.ilnbpts.org to assure that your forms were received by these deadlines. There will be no exceptions.
